Patient Record

You have the right to access which documents your patient record consists of and who has viewed them. The deadline for a response is four weeks from the receipt of the request.

Please note that for security reasons, we cannot provide copies of patient records and logs via email.

Access to patient records

All patient treatment must be documented in a patient record. You have the right to obtain a copy of your record.

Printout of log

You can request a printout of the log showing who has accessed your electronic patient record.

Restriction on access to information in patient records

If necessary to ensure proper healthcare, healthcare personnel may share your health information. You have the right to object to this. In such cases, you must inform the treatment facility.

Correction/deletion of records

Information or statements in the record may be deleted if they are incorrect, misleading, feel burdensome, or are clearly unnecessary for providing healthcare.
